HEALTH-

QUESTIONS

Health Issues

For most people, physical activity should be beneficial to their health and not cause any problems. HOWEVER, our workouts are HIGH-INTENSITY and include a variety of FULL-BODY EXERCISES with or without weights. This increases the risk of injuries and other health issues. The following 8 questions are designed to help determine whether you need a medical evaluation before starting your training. Please answer the questions below honestly. Remember, this is about YOUR health.

If your answer to one or more of questions 1–8 is "Yes," OR if you do not want to answer the questions but would still like to train with us, you have the following options:

  • Sports Medicine Evaluation: Please bring the written doctor's note with you to your first training session.

  • Disclaimer: Please fill it out, sign it, and bring it with you to your first training session.

1. Has a doctor ever told you that you have a "heart condition" and recommended that you exercise and play sports only under medical supervision?

2. Did you experience chest pain last month while at rest or during physical activity (exertion)?

3. Do you have trouble breathing at rest or during physical activity?

4. Have you ever fallen because of dizziness, or have you ever lost consciousness?

5. Do you have any bone or joint problems that could get worse with physical activity?

6. Has a doctor ever prescribed medication for high blood pressure, a heart condition, or a respiratory problem?

7. Do one or more of the following criteria apply to you?

  • Male and 40 years old or older

  • Female and 50 years old or older

  • 35 years of age or older and no regular physical activity (at least 60 minutes per week) during the past 12 months.

8. Do you know of any other reason why you shouldn't be physically active or play sports?

If you answered “Yes” to one or more of questions 1–8, you must consult a doctor to determine whether you should participate in a high-intensity fitness program. You may not train at our facility until you have obtained medical clearance or signed a liability waiver.

Sports Medicine Evaluation

If you answered "Yes" to one or more of the health questions 1–8, you must undergo a sports medicine evaluation before you can start training at bSport.

Check with your insurance provider to see if you can get a cost authorization for a sports medicine evaluation (e.g., resting and stress ECG). Your health insurance provider may cover the costs under Article 26 of the KVG, or you may be able to obtain a cost authorization through your supplemental insurance.
